The incident occurred while shooting a sequence involving gunfire, when the actors backed into an unsecured pane of glass and fell several feet.

The accident has drawn increased attention due to ongoing safety concerns raised by crew members working on the production. Issues reportedly included falling props, lack of safety protocols, and hazardous filming environments, which ultimately led to a strike organized by the International Alliance of Theatrical Stage Employees (IATSE).

Despite the incident and the ongoing labor dispute, production has continued with replacement crew members. Producers have also publicly downplayed both the accident and the concerns raised by striking workers.

As of now, Jonathan Majors has not publicly commented on the incident.
